“Living Well Chemung Offers Workshops”
Sessions begin June 5th

HORSEHEADS– The Chemung Valley Rural Health Network (CVRHN) and the Chemung County Dept. of Aging and Long Term Care are offering workshops to help participants develop skills to better manage their chronic diseases. Workshops utilize the Stanford Chronic Disease Self-Management Program which provides participants with best practices to control symptoms of chronic diseases such as high blood pressure, diabetes, heart disease, and arthritis.

The series of six workshops will be offered free of charge and will provide evidence-based (researched) ways to be healthier by learning to properly manage chronic disease. The Stanford Chronic Disease Self-Management Program has been extensively evaluated, and helps participants identify, build, and use their own skills to better manage their chronic disease.

The sessions will be held at the Horseheads Library Community Room every Tuesday, June 5th to July 10th from 5:30 pm to 7:00 pm.  Call 607-737-2826 for more information or to enroll.
